Calling all Registered Nurses (Adult, Mental Health or LD)
or Allied Health Professionals registered with the HCPC who have an interest in
supporting individuals who have an Acquired Brain Injury and/or Complex care
needs.
The Complex Case Senior Practitioner role is part of the small and friendly
Complex Care team that sits alongside the All Age Continuing Care Team in the
cluster ICB of NHS Dorset, NHS Somerset, NHS Bath, NE Somerset, Swindon & Wiltshire.
The role operates Monday to Friday core hours, with no night shifts or
weekends, with a hybrid approach of office work, remote working and face to
face assessments with individuals across the cluster footprint.

The
purpose of the role is to ensure that those requiring complex rehabilitation
and care are assessed appropriately and accepted onto an NHS funded pathway.
Practitioners will undertake review following MDT recommendation to allow for a
consideration of entry onto the funded pathways and will undertake reviews on
patients who are already in receipt of funding for complex care and Acquired
Brain Injury rehabilitation across the whole cluster region.
