We are looking for an enthusiastic and compassionate Bank
Staff Nurse to join our Living Well Services team, based at our Rowans Living
Well Centre in Purbrook.
Providing nursing support within the Living Well Services, Monday to Friday, 8:30am4:30pm, including our Community Satellite Hubs.
Supporting people who are seeking help and support before they reach the final stages of their life.
Providing opportunities to inform, support, care for and re-able people living with a life-shortening or incurable illness, as well as those approaching the end of their natural life.
Providing a welcoming environment that offers companionship and support to patients and their carers.
Promoting health, wellbeing and independence within the local community.
Supporting patients with future planning and decision-making, helping them to remain in control of their lives for as long as possible.
Assessing and triaging patients and carers and arranging appropriate programmes of care and therapy to meet their individual needs.
Using assessment tools including IPOS and CSNAT to identify needs and refer patients to appropriate professionals and therapies both within and outside the hospice.
Working collaboratively with colleagues and external professionals to ensure patients and carers receive appropriate, coordinated support.
