Physical Therapist (PT) – Home Health | Waianae, HI | Part-Time & PRN | $65–$85 Per Visit

Stern at Home Therapy is hiring Part-Time and PRN Physical Therapists in Waianae, HI,
to provide one-on-one care in patients' homes.
This is a great opportunity for a PT who wants to build a consistent local caseload or add flexible visits around an existing schedule without the double-booking, productivity quotas, or clinic politics common in traditional outpatient settings.
What You'll Do
As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout the
Waianae area
. You'll have the clinical autonomy to develop individualized plans of care while our administrative team supports you with scheduling, referrals, authorizations, and patient coordination.
Responsibilities
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments inpatients'' homes.
- Help patients improve strength, balance, mobility, endurance, and functional independence.
- Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
- Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, families, and interdisciplinary care team members.
- Manage your caseload around your availability.
Compensation
Part-Time W-2 — Minimum 10 Patients
$65 per follow-up visit.
$75 per 4-unit evaluation.
Part-Time 1099 — Minimum 10 Patients
$72 per follow-up visit.
$85 per evaluation.
PRN — Minimum 5 Patients
- $72 per follow-up visit.
- $85 per evaluation.
- Part-time and PRN compensation is paid at a fixed per-visit rate and does not use a tiered volume model.

Qualifications
- Active Physical Therapist license in Hawaii required.
- Graduate of an accredited Physical Therapy program.
- Reliable transportation required.
- Strong clinical, communication, and documentation skills.
- Ability to work independently in a home-based setting.
- Home health or geriatric experience is preferred but not required.
