Community Healthcare Assistant: Permanent (30 hours)
Our work in Primary Care Networks (PCN) is at the very heart of Stockport PCN Care. Services will be offered seven days a week to help people remain healthy and build independence. The aim being that the "right care is delivered to the right people at the right time".
Stockport PCN
is in the position to offer exciting new opportunities for Band 3 Support Workers. We are looking for enthusiastic individuals who would work with the Community Nurses and Adults Social care staff to provide a first class service to the residents of Stockport. Line management will be provided by Stockport foundation trust , under NHS terms and conditions. Candidates will be expected to complete the Care Certificate.
To assist with general wellbeing; creating a supportive and re- enabling environment promoting maximum independence. The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and Palliative patients symptom control.
