Symphony Healthcare Services are recruiting for a
Digital and Data Lead
to join the West Somerset Primary Care Network (PCN) team.
West Somerset PCN has an established central team based at
our Living Better Hub in Williton and a strong track record of collaborative
working across our practices. We are well developed in neighbourhood working,
bringing together primary care, community health, social care and voluntary and
community partners to develop services around the needs of our population.
The
Digital & Data Lead will provide practical digital, data, reporting,
communications and improvement support across the PCN. The postholder will work
closely with the PCN Manager, Clinical Director, member practices, PCN teams
and wider partners to make effective use of data and digital solutions, support
delivery of PCN priorities and help identify opportunities to improve services,
patient access and staff experience.
