The Ridge PCN are looking for an experienced general practice nurse to play a key role in supporting the health and wellbeing of residents living in care homes.
Ridge Primary Care Network (PCN) is looking for a caring, compassionate Experienced General Practice Nurse with significant post-registration expertise in frailty. The Experienced General Practice Nurse will be focusing on delivering high-quality, coordinated care to
residents in care homes across the Ridge locality. This
will include providing care from initial history taking, clinical assessment, diagnosis,
treatment, to an initial treatment plan and assistance in completing Respect
forms with the care homes and ensuring they are available on Kent Medical
Community Record [KMCR]. The post-holder work assists the nominated practice GP
to ensure compliance with the PCN Care Home Direct Enhanced Service. They will
also ensure patients within the care homes receive effective chronic disease
management.
Support
public health and screening programmes, including immunisations, vaccinations, where
appropriate.
