The Experienced General Practice Nurse plays a
pivotal role in delivering safe, high-quality, and person-centred care. This
role is integral to achieving the objectives of the 2026/27 Network Contract
DES, with a focus on continuity of care, health inequalities reduction, digital
transformation, and long-term condition (LTC) optimisation.
The postholder will work as part of the existing practice nurse
team on long-term condition management, champion public health and prevention,
and embed population health principles within general practice.
