To provide a service working within the Clinical
Nurse Specialist nursing team within the speciality of Neurology. Manages a caseload. Acting as a link with the Worcestershire
Community trust, particularly the Community Neurology Team, to ensure timely
and appropriate transfer of care in a managed way.
