About the role
Salary: £11000.00 to £11500.00
We are looking for enthusiastic and
patient-centred GPs to join our Primary Care Network (PCN) as soon as they are
able, to work within our Proactive Care Team. The role with be focussed on
helping to care for and support patients living with frailty and long term
conditions in their own homes. There is support from an experience GP with an
interest in frailty, and other AHPs with an interest and experience in frailty.
